Terms & Conditions

By booking an appointment, I acknowledge and agree to the following:

•  I understand and accept Maison Private Nursing’s cancellation, rescheduling, and refund policies, which may include non-refundable deposits and late cancellation fees.

•  I understand that a deposit or full payment may be required at the time of booking, depending on the service.

• If a payment plan is selected, I agree to complete all scheduled payments on time. Failure to pay may result in cancellation of future services.

• I understand that this appointment does not establish a nurse–client relationship until all required consent forms and clinical intake documents are completed and reviewed.

• I agree to complete all HIPAA-compliant intake forms (including consent to treat, HIPAA authorization, and service agreements) before care is rendered.

• I understand that no emergency care is provided. Maison Private Nursing is not an emergency service and does not respond to urgent or life-threatening situations. In case of emergency, I will call 911 or go to the nearest emergency room.

• I authorize Maison Private Nursing to contact me via phone, email, or text regarding appointments, services, and health-related communications.

• I agree to receive electronic communications related to scheduling, care coordination, and follow-ups.
